Concept Care is expanding, and we are seeking committed and reliable Disability Support Workers to join our growing team. We support people living with disability to live safely, independently and with confidence — and we are looking for individuals who genuinely care about the work they do.

About the Role

As a Disability Support Worker with Concept Care, you will deliver in-home and community-based support tailored to each individual’s needs.

Your responsibilities may include:

Assistance with daily living activities, personal care and medication prompts
Supporting clinical care requirements such as PEG feeding, bowel care, stoma care and catheter care
Providing transport to appointments, shopping and social activities
Implementing positive behaviour support strategies where required
Encouraging independence, skill development and community participation

This is a varied role where your consistency, presence and approach make a genuine difference.

Who We’re Looking For

We are seeking individuals who are dependable, respectful and able to remain calm under pressure.

You will be well suited to this role if you:

Communicate clearly and respectfully
Remain composed in challenging situations
Understand professional boundaries and confidentiality
Follow processes and complete documentation accurately
Work collaboratively with clients, families and team members
Are open to learning, feedback and ongoing development

Experience supporting people with complex behaviours, mental health conditions, ABI, Autism or high-support needs is highly regarded.

A Certificate IV in Disability, Aged Care or Community Services is preferred, but not essential for the right candidate.

A current Australian driver’s licence and access to a reliable vehicle are required.
